You can use the PATH Public Portal to apply for the following West Virginia DHHR programs and services:

  • Child Care Services
  • Child Support Services
  • Food & Nutrition (SNAP)
  • Health Care (Medicaid and WVCHIP)
  • Home Energy Assistance (LIEAP)
  • Medicare Premium Assistance
  • School Clothing Allowance

Using PATH, you can apply for one or more of these programs or services.

You can use the PATH Public Portal to apply for West Virginia DHHR programs and services. This tutorial will provide information about the Apply Now feature.

You must create an account or sign into an existing account to use the Apply Now feature. For information on creating an account or signing in, please see the creating an Account tutorial. After you are signed in, to begin the application process, click “Apply”.

Before starting an application, you must complete a pre-questionnaire. The pre-questionnaire provides you with instructions and information about the application process. In addition, you will answer questions, such as name, date of birth, and household members. At the top right of the application there is a toolbar with four buttons. Exit will close the application. Save, allows you to save the application to return and finish it later. Help will open in system help. And lastly, the print button prints a screen shot of the current screen. Another feature of the application is the left navigation bar, this informs you of where you are in the application process. The navigation bar changes based upon selected programs and services.

The West Virginia DHHR is committed to maintaining your privacy and security. Before completing an application, you must review and read the Privacy Notice. You may also print the notice and keep it for your records. After reading the privacy notice, click continue to advance the application and begin answering the pre-application questionnaire.

Answer each of the pre-questionnaire questions as accurately as possible. After answering all questions on a screen, you can use the Previous and Continue buttons to navigate the application.

You can apply for one or more of the available DHHR programs or services. Use the check box to select or de-select items.

To learn more about a program or service, hover the mouse over the informational icon.

Next in the pre-questionnaire is to add household members. Follow the on-screen instructions and complete the fields for all persons in the household. To add an additional member, click “Add Household Member”. Use the 'Remove' button to remove members from the application.

The top section of the screen will contain information on what will be needed to complete the questions. Mandatory questions are marked with an Asterisks, all mandatory questions must be answered. Each category of the application contains a series of questions. Answer each question as accurately as possible. The left navigation bar updates as you progress through the application. Each completed category appears in the bar with a green check mark. The check mark indicates that the category is complete and there are no errors.

The application also alerts you if there is missing or incomplete data. The category with the missing information is highlighted with a red exclamation mark. Each error in the section is listed and each error has a hyperlink to the data field.

The last steps of the application are to review and sign. To reach this step, all categories of the application must be completed and have a green check mark. Use the triangle and expand each section to review your responses for accuracy. Once all categories have been reviewed, scroll down to review rights and responsibilities.

Rights and responsibilities inform you of your rights and responsibilities for the selected program or services. You may see one or more, depending on the programs or services applied for. You can print them to keep for your records. After reviewing and or printing, you must sign the application.

Applications are electronically signed. To submit the application, you must agree that: All statements on this form have been read by me or to me and that I understand these statements. I accept the responsibilities stated above. All information I have provided is true and accurate to the best of my knowledge. I am electronically signing this application. If applying for Medicaid or CHIP you will have the option to allow DHHR to use income data from electronic sources to automatically renew Medicaid or Chip coverage. Lastly, you must type your name and date of birth. This will serve as the electronic signature. After all material has been reviewed and you have agreed and signed, the application is ready to be submitted. To submit the application, select the “Submit” button.

After submitting the application, you are presented with a message. The message informs you that the application was successfully completed and that you will receive notifications through the My Account/My Messages feature. In addition, the message provides you with a link to Contact DHHR offices and a link to manage the application. This concludes the Application tutorial.
