To complete your online application, you will be required to upload a number of important documents.Please refer to the lists below so you can gather your digital documents ahead of starting your application.


PARENTAGE/GUARDIANSHIP DOCUMENTS

  • Proof of Parentage
  • Proof of Legal Guardianship
  • Proof of Foster Care

Proof of Parentage

Documentation requirements for all incoming students
  • An official[court-issued] birth certificate.
    • Must have an official translation, if not in English.
  • State or government-issued photo ID for both parents, if two are listed on the birth certificate.
    • The parent/guardian enrolling the child must be listed on the birth certificate.
  • Marriage License or Legal Name change Document (if parent's name on birth certificate does not match name on ID or residency documents).

  • We assume shared custody unless otherwise informed. Please upload official custody documents onlyif there is a concern for safety or something we need to know for the safety of the child.
  • Legal guardianship or foster care placement plan/letter where applicable.

Proof of Legal Guardianship

  • Official, court-issued legal guardian documentation
  • An official, state-issued birth certificate.
    • Must have an official translation, if not in English.
  • State or government-issued photo ID

Proof of Foster Care

  • An officially signed foster care placement letter from case worker
    • Must have name of foster care family and address.
  • Birth certificate, if available.

If you are a homeowner

The following documents are required:

  • Most recent homestead property taxwith 100% Primary Residency Exemption (P.R.E.) i.e.
    • Most recent homestead taxes
    • Most recent tax assessment document
    • Most recent official tax print out from City office with PRE.
    • BS&A printouts arenot acceptable.
  • One most-recent homeutility bill, must have full name, full address, and date of payment.i.e.
    • DTE
    • Consumer Energy
    • Cable/Internet

If you have a land contract

  • Signed land contract by both parties.

If you are leasing property

The following documents are required:

  • Lease signedby both parties.

If you are living with a District Resident

    • Custody/divorce documents or notarized parent-release letter if the otherparent on the birth certificate doesn'tlive with the child. Must have an official translation if not in English.
  • Statement of parent/guardian living with BPS resident (form available in online application)
  • Legal guardianship or foster care placement plan/letter where applicable.

Immunization Checklist

Immunization, hearing and vision screeningrecords are not required to complete enrollment, but must be turned into the school no later than the beginning of the academic year. However, we have added a portal to our enrollment application in case you have any these records at the time of your enrollment.

As per Michigan Law (Revised School Code MCL 380.1147), students must now be 5years old by Sept.1 to be eligible for enrollment in Kindergarten.

For the school year, if a child is not automatically eligible for enrollment because s/he was born after Sept.1, but on/before Dec.1, the child’s parent or legal guardian can submit a waiver to enroll the child in kindergarten.

Student must first enroll into their zonedschool. Then the form must be completed and submitted directly to zonedschool. Applications are based on availability and approval from both principals.

Families that need access to a computer to complete the new student enrollment forms should contact the enrollment office at 248-203-3000. An onsite computer will be made available.

According to PowerSchool, the forms should work on any web browser. The technology services department of Birmingham Public Schools recommends Google Chrome or Mozilla Firefox.

What are the security measures in place to protect my electronic enrollment and registration data and documentation?
No data from electronic enrollment is stored on our district storage servers. PowerSchool Registration has a highly secure infrastructure. For details please see the detailed document below.
